IRP

 First Time IRP

-Social Security Number and/or Employee Identification Number

– Copy of the Articles of Incorporation for your company

– Copy of your CDL or copy of driver CDL if applicant does not have a valid    CDL and a copy of your driver’s license

            -Prospective employer: name, address, phone number, Safety Carrier DOT #

-Proof of ownership: Title, ST-556 Form, Bill of Sale or invoice (issued by a licensed dealer)

– Proof of address with any four of the below documents (no older than 60 days)

TITLE

Need a new title, title transfer, or need to replace or correct a title? Here’s a list of everything you will need.

Title Only

-Original title (signed by seller and buyer and date)

-Purchase price of vehicle

-Company name and address that the vehicle will be leased to (if applicable) and DOT # of the company the vehicle will drive under

Title Transfer

IFTA

IFTA is a reciprocal agreement, meaning that an IFTA license issued by the jurisdiction where the motor carrier is based, is valid in all the other IFTA member jurisdictions. Additionally, the licensee reports and pays all motor fuel taxes to the base jurisdiction, which handles distribution to all the other member jurisdictions in which the licensee travelled and incurred motor fuel use tax liability. IFTA member jurisdictions are the lower 48 states and the 10 Canadian provinces.

In order to open IFTA for your company, please provide us:

-Company name

-DOT / MC #

-FEIN # (Taxpayer ID #)

-President’s name (first and last)

-President’s Social Security number

-Correct physical address (for IFTA, no P.O. Box or UPS store address)

-Phone number and email address

-Copies of registrations for the trucks (and lease agreement if the name on the registration is different)

-Number of decal sets needed

PERMITS

The most common permits needed for the states of Kentucky, New Mexico, New York, Connecticut, and Oregon. Each state has unique requirements for when a permit should be purchased as well as how long they are considered valid. 

New York: New York requires all trucks over 18,000 pounds to renew permits every three years.

New Mexico: New Mexico requires all trucks over 26,000 pounds to renew permits annually.

Kentucky: Kentucky requires all trucks over 59,999 pounds to purchase permits only once without ever having to be renewed. If there are any changes to your inventory, this form must be updated.

Oregon: Oregon requires all trucks over 26,000 pounds to purchase permits with a bond. While carriers who frequent Oregon will want to apply for a permanent pass, one-time passes are also offered. If you’re interested in a temporary pass, please click the appropriate button below.

Connecticut: Connecticut requires all trucks over 26,000 pounds to have a valid CT Permit Certificate printed and in their trucks. Connecticut does not require each truck to be registered individually, but a CT Certificate must be in each truck going to or through the state.

HEAVY HIGHWAY VEHICLE USE TAX / 2290

A 2290 Form is the Heavy Highway Vehicle Use Tax (HHVUT). It must be filed and paid annually for every truck that drives on public highways and weighs over 55,000 lbs.

IN ORDEN TO FILED AND PAID YOUR 2290, PLEASE PROVIDE US:

-Your employer identification number (EIN). You must have an EIN to file Form 2290

-The vehicle identification number (VIN) of each vehicle

-The taxable gross weight of each vehicle to determine its category
